The Learning Assistance Center & Disability Services is responsible for (a) securing the disability documentation and information disclosed by a student seeking accommodations and (b) maintaining this information in confidential locked files separate from the student’s academic file.
Need to Know Basis
Disability information is shared with faculty and staff on a limited “need to know” basis. Common situations for this need include implementing testing accommodations or arranging accessible housing. A signed Release of Information is requested before sharing information with professors each semester. A general Authorization for Release of Information is requested before sharing disability related information to parents as applicable.
Disposal of Records
The confidential file is maintained for seven years after the student has graduated or left Wake Forest University. The file is then shredded.
